Most likely your thermostat is malfunctioning, I don't have personal experience of the problem as mine already has a manual switch for the fan.

If only we got a £1 for ever person that turned up here with overheating problems, we'd be able to buy an overheating hijet off ebay
But anyway, a more serious reply will follow:-
A good idea would be to see if the fan and its electronics work - there is a sensor in the coolant pipe which triggers the fan, if you unplug the connector and put a bit of wire across the connectors with the ignition turned on the fan should kick in if all is correct with that system.
Then you can check the sensor itself, if you unscrew it from the pipe, connector a multimeter to the two connectors then put it in boiled water from the kettle it should go from being an open circuit to a closed circuit if it works correctly.
Next idea would be to closely inspect both the radiator caps very closely and their seating seals for rust and damage, if they do not work perfectly you will have problems, they are cheap and easy to replace but make sure the seating seal on the van is good before you fit them. The one above the bumper will most likely be good while the one under the passenger seat tends to be the victim!
Next idea would be to see why the coolant system isn't working, I would first inspect the radiator and its hoses/clips very closely, you will need to remove the bumper to get a proper look which is very quick and easy.
There's a few pointers, good luck, and let us know how you get on!
